I have run out of my usual medication / I have lost some or all of my prescribed medication

Whilst we advise that all medications should be taken as prescribed, it is unusual for harm to arise if you forget or miss a single dose of your medications.

Most pharmacies will be able to loan you medications for example for a day or two until you can get a prescription from us.

This will not apply to controlled drugs such as morphine, oxycodone or gabapentin.

If you are not sure whether you need your medication urgently or if you can wait for a day, please discuss it with your pharmacist and if necessary they can contact us.

Please advise us of some more details and we will pass these to a doctor to review.

As with all medication requests, please allow 48-72 hours for this to be completed.

I have been given a new medication from a hospital consultant or other doctor

In line with our longstanding policy on the prescription of medication requested by other specialists, we will require a letter from that doctor outlining the following things:

- The name, dose and length of treatment of any medication
- The reason for the medication
- Any particular monitoring requirements for that medication.

In the case of private specialists or GPs advising medications, please note that the medication must be in the local Herts Valleys CCG NHS prescribing list in order for us to prescribe it.

If it is urgent and you have a prescription from a specialist, then you will need to use the private prescription to obtain the medication. Otherwise, you will need to wait up to 72 hours for us to issue an NHS prescription.

A copy of a private prescription is not sufficient for us to change to an NHS prescription, we require a letter as well.

I have been given the wrong quantity of medication

We are sorry that you have been given the wrong quantity of medication. Please give some more details outlining the medication/s that require an adjustment of quantity. This will be passed to a doctor to review.

As with all medication requests, please allow 48-72 hours for this to be completed.

My pharmacist says I need a review of my medication

Usually a medication review will need some blood tests. Our reception staff are now able to arrange for you to have appropriate blood tests to review your medication.

Please complete this form and our staff will be in touch informing you what is required for your individual medication review.
