Repeat Prescriptions

If you take regular medication and your doctor feels your medicines can be repeated without you being seen each time, you will normally be given a computer repeat prescription slip - a tear-off portion of your prescription - listing your medication.

Please keep this slip safely. To re-order a drug, tick the box next to it.

How to request a repeat presciption

You may either:

• request a repeat prescription using our online services
• hand the completed slip in at the reception desk, or
• send it by post
• By fax on 0844 477 2409

Please allow at least two working days for it to be processed and signed by a doctor. Remember to order well in advance of public holidays when the surgery and most chemists will be closed.

Your prescription can be posted to you if you supply a stamped addressed envelope.

Medication Review

You will be required to see a GP at least once a year for a medication review.

A message will be highlighted on your prescription counterfoil when you receive your last issue. This will give you plenty of time to make an appointment before your next prescription is due.
